How we handle your information

To reserve an appointment we collect your name, email address, phone number, whether you’re a new patient, and any notes you choose to add. We use this only to confirm your appointment, contact you about it, and prepare for your visit.

We take no payments through this website and never ask for card details online. Payment happens at the clinic, and no card information of any kind is stored on our servers.

We ask you not to send medical history through this website’s forms or by email — neither is a secure channel. Health information is collected during your screening and visit, and is protected under HIPAA and our Notice of Privacy Practices.

The referral form on this site collects practice contact details only, so our intake team can call your office back. Patient referrals containing protected health information should be sent by secure fax or by phone.

We don’t sell your information. We share it only with service providers that help us operate — currently our website host and our email provider — and where required by law.
