How to refer a patient
Send the referral and we take it from there — benefits verification, prior authorization, scheduling, and a report back to your office after each visit.

Fax the form and chart notes
Send the completed form to (916) 678-5839 with demographics, the insurance card, recent labs, and relevant chart notes.

Or call our intake line
Call (916) 678-5840 Monday–Friday, 8:00 AM – 4:00 PM and our intake coordinator will take the referral over the phone.

We handle the rest
Benefits verification, prior authorization, financial assistance, scheduling the series — and a report back to your office after each administration.
